• Identification: Please bring a valid form of identification, such as your Medicare card.
   
• Medical history: Details of any existing medical conditions (e.g. high blood pressure, diabetes), previous surgeries, and relevant family history of heart disease.
   
• Current medications: A list of all medications you are currently taking, including dosages and frequency.
